TELEPHONIC ORAL PROFICIENCY INTERVIEW (OPI) APPOINTMENT FORM

Dear Applicant:

Using the links below, please complete the application form, with credit card payment, and forward it, on line, with all required forms, to the ACTFL Language Testing Office. We will then contact you as soon as possible to schedule your Oral Proficiency Interview.

It will take up to four weeks from the time of the actual interview to issue the official ACTFL proficiency rating. We will send you a letter and certificate stating the rating, as soon as possible.

Once we receive your application, we will match your availability with that of a tester in the language you indicate and set up an appointment with the tester. We will then contact you to advise you of the appointment date and time, name of the tester and a telephone number to call for the interview. If for any reason you need to reschedule an appointment, please contact us immediately. If you miss an appointment without contacting our office at least 24 hours in advance, you will be charged a no show and rescheduling fee of $55.00.

If you wish to have the official results mailed to you as well as your online, printable version, please complete the section entitled 'Send Official Copy' in the application form below, for an additional fee of $10.00.

NOTE: For a telephonic oral assessment, ACTFL requires that a person in an official school capacity place that call and introduce the candidate to the tester at the beginning of the interview to verify the candidate's identity. Please indicate the name, title, telephone number, and fax number of this person (The Test Administrator) on the application form.
